Health Information

Please let the nurse know if your child has any change in their health status.
Please update the Emergency Information for your student in Infinite Campus if any information has changed.
In order for the Nurse to give your child over the counter pain medicine, parent permissions must be updated every school year.


Current Immunization Requirements: PA Immunization Rules for Back to School

PA State Immunization Requirement for all 7th grade students: 1 dose of tetanus/diphtheria/acellular pertussis (Tdap) on the first day of 7th grade and 1 dose of meningococcal conjugate vaccine (MCV) on the first day of 7th grade. 
Request a copy of your child's immunization record anytime your child gets immunizations at their provider and please send it in to the School Nurse.

 

PA State Screening Requirements:

If you do not wish for your child to be screened by the school nurse for these mandated requirements, please email or call your Certified School Nurse.


Medications:

Any medication must be distributed through the nurse, including over-the-counter analgesics.  If you need to send in a medication prescribed by a physician, please complete the school medication form in which both the prescribing doctor and parent must sign.

  • Only a parent/guardian/adult can drop off medication to the school nurse in the original medication packaging.
  • Cough Drops: Per new school policy in 2018, the school nurse can not administer to students as this is not a listed medication/doctor order.
  • Epi Pen Shortage: Due to a production shortage of Epi Pens, the FDA has extended Epi Pen expiration dates. Please check FDA website for more information.
